Carleen Conde is a Board-Certified Family Health Nurse Practitioner dedicated to providing comprehensive, compassionate, and patient-centered care. With more than 13 years of clinical experience at a Level 1 Trauma Hospital Center, she has developed a strong foundation in managing complex health conditions in fast-paced and high-acuity environments. Her decade-long journey in nursing has shaped her into a meticulous, knowledgeable, and deeply committed healthcare professional.

Carleen began her career as an Operating Room Nurse, spending six years working closely with multidisciplinary teams in the demanding field of Vascular Surgery. Her experience in the OR strengthened her expertise in surgical care, critical clinical decision-making, and the importance of precision and teamwork. This background instilled in her a deep appreciation for detail-oriented practice and the collaborative nature of high-quality patient care.

Transitioning into her role as a Family Health Nurse Practitioner allowed Carleen to broaden her scope of practice to include primary care, health promotion, and patient education for individuals across the lifespan. Drawing from her trauma and surgical experience, she brings a calm, structured, and thorough approach to every patient encounter. Carleen is passionate about delivering holistic care that supports the physical, emotional, and social well-being of every patient she serves.

In addition to her clinical work, Carleen is a full-time Nursing Clinical Instructor. Her commitment to education is reflected in her dedication to mentoring the next generation of nurses. She creates dynamic, supportive learning environments that promote critical thinking, empathy, and professional growth. Teaching allows her to share her real-world experience while fostering the skills and confidence her students need to thrive in diverse healthcare settings.
